29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about April 11, 2013: (a) Employees performing roofing operations on a roofing system with a pitch of 8 in 12 were exposed to fall hazards of approximately 12 feet, without the use of any fall protection measures.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 11, 2013: (a) Employees were exposed to fall hazards of approximately 12 feet while using an aluminum extension ladder to access the upper landing area of the roofing system, where the rails of the ladder did not extend at least three feet above the landing area.
