29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:   a) 101 W. Airline Hwy., Laplace, LA; On or about 4/17/13, the employer allowed employees to work from a scaffold level that was not fully planked. The employees were 12 feet 10 inches above the ground.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   a) 101 W. Airline Hwy., Laplace, LA; On or about 4/17/13, the employer did not ensure that employees working from a tubular welded scaffold 12 feet 10 inches above the ground was protected by a complete guardrail system. The scaffold was equipped with a midrail approximately 24 inches high but no toprail.

Employees using hand and power tools and exposed to the hazard of falling, flying, abrasive, and splashing objects, or exposed to harmful dusts, fumes, mists, vapors, or gases shall be provided with the particular personal protective equipment necessary to protect them from the hazard. All personal protective equipment shall meet the requirements and be maintained according to Subparts D and E of this part.   a) 101 W. Airline Hwy., Laplace, LA; On or about 4/17/13, the employer allowed an employee to operate a Dewalt side grinder to install sheet metal while not wearing a face shield for protection from the flying sparks.
