Burkholder Drywall LLC worksite located at 4753 Carriage Crossing, Sylvania, Ohio: On or about April 15, 2013, the employer failed to utilize base plates at the bottom of the scaffolding system to ensure a firm foundation.  The legs of the supporting frame were bearing directly on the ground.

Burkholder Drywall LLC worksite located at 4753 Carriage Crossing, Sylvania, Ohio: On or about April 15, 2013, the employer did not ensure uplift pins or equivalent means were used on the tubular welded frame scaffold while workers performed masonry operations on the east side of the two story residential structure.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.      A. Burkholder Drywall LLC worksite located at 4753 Carriage Crossing, Sylvania, Ohio: On or about April 15, 2013, the employer failed to ensure employees were protected from fall hazards while performing masonry activities on the East side of the two story residential structure. Employees were exposed to fall distances up to approximately 21.00 feet.    Burkholder Drywall LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451, which was contained in OSHA inspection number 311604490, citation number 1, item number 2 and was affirmed as a final order on November 25, 2008 with respect to a workplace located at Sylvania, Ohio.
