Both forward and aft access ladders were blocked by steel coils and tractor tires.

Onboard M/V BLUE OCEAN, hatch five: An employee was operating a Mitsubishi forklift on which the seatbelt was not attached to the brackets on the seat where it would be installed.

Onboard M/V BLUE OCEAN, hatch two: The employer did not ensure employees stayed clear of the area beneath overhead drafts.  Employees working in the hatch remained in an area beneath the load where they could be struck by tires being hoisted out of the hatch in slings were to part.

29 CFR 1904.32(b)(2)(i): Total numbers of the columns on OSHA 300 Log did not match total numbers on the Annual Summary:    a) Stevedoring Services of America: The employer did not ensure at the end of the year that entries were complete and accurate and an annual summary of injuries and illnesses on the OSHA 300 Log was created, certified and posted.  The OSHA 300 A for the year 2010 reflected forty seven days away from work (K) while the OSHA 300 log reflected one hundred fifty two days away from work.    b) Stevedoring Services of America: The employer did not ensure at the end of the year that entries were complete and accurate and an annual summary of injuries and illnesses on the OSHA 300 Log was created, certified and posted.  The OSHA 300 A for the year 2011 reflected five injuries (M1) while the OSHA 300 log reflected six injuries. The OSHA 300 A reflected  one hundred and one  days away from work (K) while the OSHA 300 log reflected three hundred and eight days away from work.    c)  Stevedoring Services of America: The employer did not ensure at the end of the year that entries were complete and accurate and an annual summary of injuries and illnesses on the OSHA 300 Log was created, certified and posted.  The OSHA 300 A for the year 2012 reflected  two hundred forty two days away from work (K) while the OSHA 300 log reflected five hundred ninety four days away from work.
