The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a. Throughout Facility- On or about 15 April, 2013, the employer did not establish an energy control program for maintenance employees performing maintenance on facility equipment, specifically, Lathe type 1340, serial number 17745, thereby exposing employees to the hazard of unexpected energization of equipment.

On or about April 15, 2013 in the facility, employees were required to operate a Clark Electric NSN powered industrial vehicle in need of repair; the lights of the machine were inoperable, steering column was cracked, and backup alarm was not functioning, thererby exposing the employees to struck by hazards.

This exposure shall begin at a point not less than 15 deg. below the horizontal plane of the wheel spindle. a. Machine Area- On or about April 15, 2013, there was no guard affixed to Brown and Sharp surface grinder serial number 11774, thereby exposing the employees to struck by hazards. b. Machine Area- On or about April 15, 2013, there was no guard affixed to Brown and Sharp surface grinder serial number 2536180-2407, thereby exposing the employees to struck by hazards.
