29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the entries were accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year.  The Shaw Group, Inc. - Shaw Sunland Fabricators, Inc. (Walker, LA) did not record the following workplace injury correctly on the OSHA 300 Log for calendar year 2012:     a)  The employer does not ensure that entries on the OSHA 300 log are complete and accurate or corrections are made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury of employee SUN-12-003 (Shipping Loader) was recorded incorrectly on the OSHA 300 log for 2012 as thirty-two (32) days of job transfer or restriction.  The employee crushed his right small finger while loading pipe spools onto a truck causing a fracture and laceration, resulting in at least eighty-four (84) days of job transfer or restriction, on January 12, 2012.      b)  The employer does not ensure that entries on the OSHA 300 log are complete and accurate or corrections are made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury of employee SUN-12-014 (Maintenance manager) was recorded incorrectly on the OSHA 300 log for 2012 as seventy-seven (77) days of job transfer or restriction.  The employee suffered cut his left index finger with a skill saw while building a storage cabinet inside the maintenance shop, resulting in at least one (1) day away from work, on December 11, 2012.
