MassDOT facility, 811 North King Street, Northampton, Massachusetts:  29 CFR 1926.305(c): When it was necessary to provide a firm foundation, the base of the jack was not blocked or cribbed; a wood block was not placed between the cap and the load where there was a possibility of slippage of the metal cap of the jack:   Instance a:  On or about April 18, 2013. the bases of the hydraulic bottle jacks were not sufficiently blocked and cribbed to provide a firm foundation while blocking and leveling a William Scotsman modular building.  Instance b:  On or about April 18, 2013, there were no wood blocks between the caps of the jacks and frame of the trailer to prevent slippage of the jacks.

29 CFR 1926.305(d)(1)(i): The load was not cribbed, blocked, or otherwise secured at once after the load had been raised:    MassDOT Facility, 811 North King Street, Northampton, Massachusetts:    On or about April 18, 2013, the mobile office trailer was not properly cribbed to prevent the load from shifting in the event of a jack failure.
