29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards:   Location: 177 Highland Street, Worcester, MA. On or or about 4/18/2013 Employees doing roofing work were exposed to fall hazards greater than 25 feet and had not been trained on fall protection by the employer to recognize the hazards of falling and procedures to be followed to minimize these hazards.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed:   Location: 177 Highland Street, Worcester, MA. On or about 4/18/2013 employees used a 6 foot step ladder to access the roof from the attic of the residence which did not extend any length above the upper landing surface.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure:   Location: 177 Highland Street, Worcester, MA. On or about 4/18/2013, employees were observed installing shingles on a roof more than 25 feet above the ground below without any fall protection:   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
