Whenever practicable, all arc welding and cutting operations shall be shielded by noncombustible or flameproof screens which will protect employees and other persons working in the vicinity from the direct rays of the arc.    Location: 299 Broadway, New York, NY.    a) Employer failed to provide adequate means of shielding for employees while cutting and torching steel I-beams.  Employees were exposed to excessive rays from the arc; on or about 4/17/13.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     Location: 299 Broadway., New York, NY.     a) Employer failed to ensure that employees were protected from falling while cutting steel I-beams and while walking and/or working near an unprotected edge at the site. The employees were exposed to a potential fall of up to approximately 11 ft. to the floor below; on or about 4/17/13.
