29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Residential construction job site, 1500 Block of Federal Street, Pittsburgh, Pennsylvania, 15212: Observed on or about March 12, 2013, an employee engaged in framing activities on the third floor of a residential dwelling was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. The employee was exposed to a fall hazard of approximately 20 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: a) Residential construction job site, 1500 Block of Federal Street, Pittsburgh, Pennsylvania, 15212: Observed on or about March 12, 2013, employees were not trained in the recognition of fall hazards and the procedures to be used to reduce or eliminate these hazards. An employee engaged in framing activities on the third floor of the structure was exposed to a fall hazard of approximately 20 feet to the ground below.
