29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6): (a) Jobsite: On or about April 16, 2013, an employee was working form a ladder jack scaffold and the one aluminum ladder was only about 14 inches form the overhead energized 120/240 v AC insulated electrical line. According to the table in subparagraph (f)(6), the minimum distance for the ladder from the overhead electrical line is 3 feet.

The fall hazard was approximately 19 feet to the gorund.
