29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.       a) Behind the house, south side:       Employees were operating an overhead hoist motor to hoist a single-point suspension scaffold and a section of metal ventilation duct. An employee who was working from the scaffold platform was not protected by a protective helmet while operating the hoist. Both the scaffold and the duct were connected to the hoist and anchored to a roof brick chimney, which was overloaded and collapsed into brick pieces in an accident. The employee was struck by a brick piece fell off the roof and suffered head injury. On or about 04/15/13.       NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM.
