29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the entries were accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year. The Shaw Group, Inc.  AEP Turk (Fulton, AR) did not record the following workplace injuries correctly on the OSHA 300 Log for calendar year 2012:     a) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury was recorded incorrectly on the OSHA 300 log for 2012 as an Other Recordable case for A-12-005 (Welder). The employee injured the left knee/thigh on February 21, 2012 while walking to the print shack and stepping on a rock resulting in discomfort to the left knee/thigh. The injury resulted in restricted work activity for at least 5 days.       d) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified.  This violation occurred on or about December 31, 2012, when an injury was recorded incorrectly on the OSHA 300 log as five (5) job transfer or restricted days. The employee A-12-002 (Pipefitter) injured the right thumb while assisting a co-worker to unload a piece of 3/4 inch plywood from a forklift on June 20, 2012 causing discomfort in the right thumb resulting in sixty (60) days of job transfer or restriction.  A corticosteroid injection and a prescription for ibuprofen 800 mg were also given.
