29 CFR 1910.1030(h)(1)(ii)(B): The employer did not establish and maintain an accurate record for each employee with occupational exposure which includes a copy of the employee's hepatitis B vaccinations and any medical records relative to the employee's ability to receive vaccination as required by paragraph (f)(2) of the OSHA Bloodborne Pathogens Standard.       a) Cardiac & Thoracic Surgery - The employer did not ensure an accurate record that their employees had completed the vaccination series recommended by the U.S. Public Health Service. Two (2) physicians in the practice had not completed the series of three (3) shots or did not sign a declination form. This was observed on April 22, 2013.
