29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18 inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end. 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.    (a) Location - Bell Rock Street, Malden, MA:    On or about 4/22/13, the employer failed to ensure that each employee working on a scaffold more than 10 feet above a lower level was provided with fall protection.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     (a) Location - Bell Rock Street, Malden, MA:    On or about 4/22/13, the employer failed to ensure that each worker engaged in roofing work 6 feet or more above a lower level was provided with fall protection.      The Jospeh S. Savini company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 313821183, citation number 1, item number 2, and was affirmed as a final order on October 26, 2009, with respect to a workplace located at 76 Walnut Street, Lynnfield, MA.
