VAMC Boneyard, Asheville, NC: On or before 4/17/2013, employees engaged roofing activities on a 4/12 pitched roof were not protected from fall hazards greater than 14 feet.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:    VAMC, Boneyard, Asheville, NC:  On or about 4/17/2013, the employer did not ensure that a SafeWaze roofing anchor being used to support fall protection devices was secured properly thereby, exposing employees to a potential fall hazard of approximately 14 feet.
