29 CFR 1910.333(b)(2)(i): The employer did not maintain a written copy of the procedures outlined in paragraph (b)(2) of 29 CFR 1910.333 and did not make it available for inspection by the Assistant Secretary of Labor and his or her authorized representatives:   On or about April 23, 2013 the employer did not maintain a written copy of lockout procedures for de-energizing fixed electrical equipment, including radiological training equipment.
