29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: 773 Durham Road, Worksite, Roof on or about April 4, 2013, employees are conducting roofing work on a steeply pitched roof, greater than six feet above grade, and are accessing the roof fields from a ladder than is not extended at least three feet above the landing surface nor is it tied off to prevent displacement.
