29 CFR 1926.451(f)(4): Any part of a scaffold damaged or weakened such that its strength is less than that required by paragraph (a) of this section shall be immediately repaired or replaced, braced to meet those provisions, or removed from service until repaired: On or about April 25, 2013, employees doing roofing installation were working on a tubular welded scaffold two tiers in height approximately 18 feet high that had defective brakes on two wheel casters exposing them to a fall hazard.

29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: On or about April 25, 2013, employees doing roof installation working on the second level tier level of a scaffold approximately 18 feet high were using a Louisville 16-foot extension ladder and a Werner 6-foot self-supporting step-ladder to access a roof of a construction building exposing them to a fall hazard.
