29 CFR 1926.451(g)(4)(iii): When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform. On or about April 29, 2013 at the worksite; employees were operating and working from mobile scaffolds (i.e. scissor lift) without the midrail chain connected.
