29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about April 25, 2013, employees were exposed to falls of approximately 20 feet to the ground level below, when working on the roof of a two story residential home with no fall protection, while installing the asphalt shingles.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: On or about April 25, 2013, employees were exposed to falls of approximately 15 feet to the ground level below, while using a Werner 32 feet extension ladder, which extended approximately 2 feet beyond the landing, while accessing the roof of the residential home.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: On or about April 25, 2013, employees were exposed to falls of approximately 15 feet in height to the ground level below, when using a Werner 32 feet extension ladder with the rope missing, that is used for extending the sections, while accessing and egressing the roof.
