29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about April 25, 2013, at the above mentioned jobsite, an employee was conducting roofing work at a height of approximately 14 feet without a means of fall protection.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:    On or about April 25, 2013, at the above mentioned jobsite, employees were accessing the roof of a church with a ladder that was not secured.
