On or about 25 April 2013, and at times prior thereto, employees had not received either practical or on the job evaluations for the forklifts utilized at the work site and were only given web-based forklift training, exposing the operators and other employees working nearby to the hazard of being struck by the forklift or the load being carried.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure employees are receiving both a practical and on the job forklift evaluation before the driver is allowed to operate the truck.
