29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection.   a)  52 Main Street, Manchester, CT. Job Site - On or about 4-25-13 - Employees were observed using extension cords to power portable hand operated electrical equipment with no GFCI in the system which exposed them to shock and burn hazards.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:   a)  52 Main Street, Manchester, CT. Job Site - On or about 4-25-13 - Employee was observed in a JLG aerial lift cleaning windows using a personal fall arrest system that was not rigged properly.  The shock absorbing lanyard in use would allow the employee to hit the ground or roof surface before stopping the fall.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:  a)  52 Main Street, Manchester, CT. Job Site - On or about 4-25-13 - Employees were exposed to fall hazards of contacting lower levels from 6 to 16 feet by the use of improperly rigged personal fall arrest systems.  The employees were not trained how to rig the shock absorbing lanyards to calculate the fall distance to lower surfaces.
