29 CFR 1926.451(b)(1): The employer did not ensure that all working levels of the scaffold was fully planked or decked between the front uprights and the guardrail support: (a) Central Rockies Plastering, Inc. @ 1015 Wildwood Rd., Avon, CO 81620: On and before 4/24/13, the creating employer did not ensure that the working levels on the south fabricated frame scaffold systems were fully planked. The working levels were observed with two, rather than three, 2x12 in. wooden planks. This condition exposed a contracted employee to an approximate fall hazard of 12 ft. (b) Central Rockies Plastering, Inc. @ 1015 Wildwood Rd., Avon, CO 81620: On and before 4/24/13, the creating employer did not ensure that the working levels on the north fabricated frame scaffold systems were fully planked. The working levels were observed with two, rather than three, 2x12 in. wooden planks. This condition exposed a contracted employee to an approximate fall hazard of up to 25 ft.

29 CFR 1926.451(c)(1)(ii): The employer did not ensure that guys, ties, and/or braces were installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 ft. (6.1 m) or less thereafter for scaffolds 3 ft. (0.91 m) wide or less: (a) Central Rockies Plastering, Inc. @ 1015 Wildwood Rd., Avon, CO 81620: On and before 4/24/13, the creating employer did not install guys, ties, and/or braces to secure the south fabricated frame scaffold system which measured 3 ft. in width and approximately 25 ft. in height. The contracted employee worked up to the second level and so, was exposed to a fall hazard of approximately 12 ft. (b) Central Rockies Plastering, Inc. @ 1015 Wildwood Rd., Avon, CO 81620: On and before 4/24/13, the creating employer did not install guys, ties, and/or braces to secure the north fabricated frame scaffold system which measured 3 ft. in width and approximately 25 ft. in height. The contracted employee worked on all four levels and so, was exposed to a fall hazard of approximately 25 ft.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar structure was not used: (a) Central Rockies Plastering, Inc. @ 1015 Wildwood Rd., Avon, CO 81620: On and before 4/24/13, the creating employer did not equip the north fabricated frame scaffold system with ladder access. This condition required the contracted employee climb the end-unit frames to access the working levels. This condition exposed the contracted employee an approximate fall hazard of 25 ft.
