The large autoclave had multiple hazardous energy sources, including electrical power, pressure, compressed gas (propane), and hydraulic energy.  The energy control procedure for the large autoclave did not address the door of the autoclave, powered by electricity and hydraulic energy.  This condition exposed employees to an uncontrolled hazardous energy source.
