29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a. On or about April, 24 2013 at the above addressed jobsite, three employees were using a portable extension ladder to gain access to a roof top, which did not extend at least three feet above the upper landing surface.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a. On or about April, 24 2013 at the above addressed jobsite, three employees were  observed installing/removing roofing material from a low-slope roof without the use of conventional fall protection or without a warning line system and safety monitoring system.    REPEAT: Lou Jezdimir Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR. 1926.501(b)(10, or its equivalent standard, which was contained in OSHA inspection number 787981, citation number 01, Item number 001, and was affirmed as a final order on 04/25/2013, with respect to a workplace located at 430 Lido Drive Fort Lauderdale Florida.
