29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):      a)    Jacaranda Beach Villas Elevators - The machine specific lock out tag out procedures in place for the elevators were not written, did not contain specific procedural steps for the placement and removal of lock out devices and did not contain specific requirements for testing the elevators to determine lock out effectiveness. This exposed employees of a contractor to a caught-in hazard, on or about April 24, 2013.

This exposed the employees of a contractor to a caught-in hazard, on or about April 24, 2013.
