On or about April 26, 2013; a). Employees working in the cellar area of the project in close proximity to the live 240 volt electrical service panel and meters.

However, rosettes and cleat-type lampholders and receptacles located at least 8 feet (2.44 m) above the floor may have exposed parts. Location; 68 West 119th Street, New York, NY 10026, Basement level On or about April 26, 2013; a). Employee installing handrails, stair guards and other miscellanious interior renovations were exposed to electrical shock and or electricution because electrical outlets and switch devices were not covered or secured to the junction boxes to prevent employee contact with exposed live wires.

29 CFR 1926.501(b)(4)(i): Each employee on a walking/working surface was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around said hole (Stairway to floor below) Location 68 West 119th Street, Basement, Cellar and First Floor Levels On or about April 26, 2013; a). Employees without personal fall protection, were installing hand rails, guardrails and miscellanious interior renovations on the basement level of the above location. An 8 foot by 8 foot hole for the stairway access to the lower level 8 feet below, was not protected by covers or guardrails to prevent persons falling to the floor below.
