29 CFR 1926.300(b)(2): Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment shall be guarded if such parts are exposed to contact by employees or otherwise create a hazard. Guarding shall meet the requirements as set forth in American National Standards Institute, B15.1-1953 (R1958), Safety Code for Mechanical Power-Transmission Apparatus. (a) Back of Truck - On or about April 17, 2013 an employee was exposed to getting fingers amputated while using an air compressor with unguarded belts and pulleys.

(a) Back of House - On or about April 17, 2013 two employees were exposed to a fall hazard while working on a wooden fabricated scaffold with walkway less than 18 inches. The scaffold walkway was constructed of a 7 inch wide board in one area and a 9 inch wide board in another area.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. (a) Construction Site - On or about April 17, 2013 the employer had not train each employee using ladders on how to recognize, prevent and eliminate fall hazards associated with employees using ladders.
