On or about April 24, 2013, on the 3:12 pitch roof, an employee conducting roofing work was not adequaterly protected from a 33 feet fall hazard in that he had his lanyard attached to a non-locking snaphook.

On or about April 24, 2013, on the 3:12 pitch roof, an employee conducting roofing work was wearing his fall protection equipment improperly by having it connected to an attachment point at his waist, exposing himself to a 33 feet fall hazard.

On or about April 24, 2013, the employer did not train his employee about using fall protection or the hazards associated with falls.
