29 CFR 1915.13(b)(7): A competent person did not test ventilation discharge areas and other areas where vapors may collect, to determine if vapors discharged from the spaces being ventilated were accumulating in concentrations hazardous to employees. a) Kirby barge 28194 - On April 24, 2013: Employees were exposed to hazardous concentration of vapors, when the six tanks containing natural gasoline residuals were being ventilated with pneumatic blowers placed on the open port holes of each tank. The employer did not determine the concentration of hazardous vapors in the discharge area of the vessel. b) Kirby barge 28182 - On April 24, 2013: Employees were exposed to hazardous concentration of vapors, when the six tanks which containing natural gasoline residuals were being ventilated with pneumatic blowers placed on the open port holes of each tank. 29 CFR 1915.157(a): The employer did not ensure that employees used appropriate hand protection or protective clothing when exposed to hazards such as skin absorption of harmful substances, severe cuts, lacerations, abrasions, punctures, chemical burns, thermal burns, harmful temperature extremes, and sharp objects. a) Kirby Barges 28194 and 28182 located at the 200 Dunlap Road marina - On April 24, 2013 employees were exposed to flash fires while working in an area containing flammable vapors and the employer provided employees clothing that was made of 65% Polyester and 35% cotton. The International Safety Guide for Oil Tankers and Terminals section 26.2.4 states clothing made of synthetic materials is not considered suitable for employees where they may be exposed to flames or hot surfaces in the course of their duties.
