29 CFR 1926.20 (b)(2) The employer did not develop an accident prevention program, that provided for frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons designated by the employer.   a)  Bay Avenue & Campbell Street, Union Beach, NJ:  The employer did not designate a competent person to identify and address the hazards of the job site such as but not limited to fall hazards, observed on or about 4/25/13.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501 (b)(13) Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    a)  Bay Avenue & Campbell Street, Union Beach, NJ:  Employees were exposed to fall hazards of greater than 20 feet from the eave to the ground level.  Employees were removing and replacing a roof on a residential home, observed on or about 4/25/13.
