29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    (a) Throughout the plant. On or about April 30, 2013, employees operating forklifts did not receive initial training in safe operation of the powered industrial vehicles exposing themselves and others to struck-by hazards.    Abatement certification is required of this item in accordance with the requirements of 1903.19(c).
