These requirements are in addition to any other requirements for equipment grounding conductors. Location: 70 West 119th Street, New York, NY 10026, throughout project. On or about April 26, 2013; a). Employees were using portable screw gus, and circular saws without ground fault circuit interupters or an assured grounding program.

Electrical outlets and switch devices were not covered or properly secured to the junction boxes to prevent employee contact with exposed wires.

29 CFR 1926.501(b)(4)(i): Employees on a walking/working surface were not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around said hole (Stairway to floor below) Location 68 West 119th Street, Basement, Cellar and First Floor Levels On or about April 26, 2013; a). Employees were working on the beasement level of the above location installing sheetrock and metal studs. The 8 foot by 8 foot hole for the stairway access to the lower level 8 feet below, was not protected by guardrails to prevent workers from falling to the floor below.
