29 CFR 1926.451(b)(2)(i): Each ladder jack scaffold, top plate bracket scaffold, roof bracket scaffold, and pump jack scaffold was not at least 12 inches (30 cm) wide:  a) Employees were working from ladder jack scaffold platforms that were less than 12 inches wide.  The Forest Park Stucco, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(2)(i), which was contained in OSHA inspection number 313147167, citation number 1, item number 2 and was affirmed as a final order on June 25, 2009, with respect to a workplace located at 634 Grove Ave. Oak Park, IL.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) Employees were exposed to the hazard of falling while working from ladder jack scaffold platform elevated approximately 13.8 feet above the ground.  The Forest Park Stucco, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(i), which was contained in OSHA inspection number 313147167, citation number 1, item number 3 and was affirmed as a final order on June 25, 2009, with respect to a workplace located at 634 Grove Ave. Oak Park, IL.  The Forest Park Stucco, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(i), which was contained in OSHA inspection number 313622003, citation number 2, item number 1 and was affirmed as a final order on September 3, 2009, with respect to a workplace located at 300 Scoville Ave. Oak Park, IL.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
