29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  On or about 04/24/2013 at the 699 Enfield Street Construction Project,  an employee was exposed to electrical hazards while using a circular saw with no ground fault circuit interrupter protection in use.  The company did not have an assured equipment grounding conductor program.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  On or about 04/17/2013 at the 699 Enfield Street Construction Project, an employee was using an extension cord to energize a circular saw and the extension cord was missing the ground pin.
