29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Jobsite, Carpenter Square, Unit 1021 3rd Floor Deck - Employees were performing plumbing operations within 3' of an unprotected 19' wide by 6'6" long deck 29' above ground level without fall protection, observed on or about 4/25/13.    b) Jobsite, Carpenter Square, Unit 1021 - Employees were performing plumbing operations within 2' of an 3'8" wide by 3'4" long floor hole 12' above the next level without fall protection, observed on or about 4/25/13.

The employee did not know the height ladders are to extend above the landing surface on or about 4/25/13.
