29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a. Butcher area - On or about May 1, 2013, the employer had not implemented an energy control program for employees who perform servicing on the meat cutting band saw.  Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.

Butcher area - On or about April 16, 2013, an employee was injured while operating with an inadequately guarded meat cutting band saw.  The employees were thereby exposed to the hazard associated with unguarded points of operation.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action or overheating.     a. Butcher area - On or about May 1, 2013, the electrical box connector of the meat cutting band saw was damaged allowing exposure of electrical wiring.  Employees were thereby exposed to the electrical hazards associated with live parts.
