Throughout facility - On or about May 1, 2013, the employer did not establish a lockout/ tagout program, consisting of the following:  written procedures, periodic inspections or employee training, thereby exposing employees to amputation hazards due to the potential for unexpected start-up of serge machine, fusing machine, riveting machine, and other operating equipment.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    Throughout facility - On or about March 14, 2013, the employer was bypassing a two hand operation with a timer on a GEO. Knight & Co. Inc., embossing machine, model SP, serial number 366A, thereby exposing employees to the point of operation resulting in a finger amputation.
