29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Injection Molding Dept., on or about 5/1/13: Point of operation at the cutters and ingoing nip points at the conveyor belt and pulleys on a degating machine were not adequately guarded. b) Maintenance Area, on or about 5/1/13: Rotating chucks on a Tradesman drill press, a Bridgeport milling machine and a Craftsman lathe were not guarded. c) Brush Room, on or about 5/1/13: Point of operation on two United Solutions hot stamp machines were not adequately guarded in that you could reach under and around the guard. d) Brush Room, on or about 5/1/13: Point of operation on Inkprint 60 graphic printer was not adequately guarded in that the side guards were not in place. e) Brush Room, on or about 5/1/13: Point of operation, ingoing nip points and pinch points on three Boucherie bristle machines #1, 2 and 3 were not adequately guarded. f) Bench Assembly, on or about 5/1/13: Point of operation on a Visual Thermoform blister pack machine was not adequately guarded. g) Bench Assembly, on or about 5/1/13: Point of operation on three electric knife rope cutters were not guarded. h) Bag Converter Area, on or about 5/1/13: Point of operation on an Amplas bag converter #1896 was not guarded. Abatement certification must be submitted for these items.
