29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about April 30, 2013 at the above address an employee was performing work on the ground below a roof that employees were tearing off. The employee was not wearing a hardhat and exposed to falling debris.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:    On or about April 30, 2013 at the above address employees were performing roofing work. Employees were not tied off and were exposed to a fall hazard of approximately 10.4 feet.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
