29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501.     On or about May 1, 2013 at the above address employees were working on a residential roof and were not utilizing fall protection. Employees were exposed to a fall hazard of approximately 10.3 feet.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.    On or about May 1, 2013 at the above address employees were accessing the roof by an extension ladder that was not three feet above the landing. The employees were exposed to fall hazard while accessing the roof.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903(c).
