29 CFR 1910.23(e)(2): Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with:    A) Fixed metal stairs to auto stick placer machine, on or about May 1, 2013, the top rail to the stair rail system was approximately 49 inches high, exposing employees to a fall hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    A) Stacker machine, on or about April 23, access to the auto stick placement machine where the stick are pushed horizontally by metal forks was not guarded allowing employee to easily walk into the area, exposing employees to struck by hazards.
