29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  A) McDonald's Store #25184, Georges Road and Milltown Road, North Brunswick, NJ 08902: Employee was exposed to fall hazards of approximately 8' from the ground below while working in a Genie S-40 Aerial Lift installing performing framing operations without a fall protection system in place.   Condition occurred on or about May 2, 2013.
