WORKSHOP: The hazardous energy control lockout/tagout procedures were generic and not written step-by-step procedures in machine specifics for the employees during servicing and maintenance of equipment, such as (but not limited to) the grinders.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. WORKSHOP: The employes, as authorized were not provided with adequate training and information in the controlling of potentially hazardous and unexpected releases of energy sources (lockout/tagout) during servicing and maintenance of equipment, such as (but not limited not limited to) the grinders. ABATED DURING THE INSPECTION.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. WORKSHOP: A. Packing Department: The rotating chuck of the SouthBend lathe was unguarded. ABATED DURING THE INSPECTION. B. Grinding Department: The ingoing nip point, created by the sand belt and wheel located at below the work rest of the 967 vertical belt sander, unknown identification was unguarded. ABATED DURING THE INSPECTION.

29 CFR 1910.134(k)(6): The basic advisory information on respirators, as presented in Appendix D of this section, shall be provided by the employer in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer. WORKSHOP: The employer had not provided the basic information contained within the Appendix D of OSHA regulatory standard to the employees who were optionally wearing filtering face pieces respirators.
