29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level.   Location: Republic and 15th Street, Cincinnati, OH  a) On or about 2 May, 2013, an employee was performing masonry work on the exterior of a structure from a self-supported welded tubular scaffold system.  The scaffold system was not protected by standard guardrails and the employee was not protected by a personal fall arrest or restraint system.  The employee was exposed to a potential fall in excess of 16 feet.   Speed Maintenance Service, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(vii), which was contained in OSHA inspection number 313011389, citation number 1, item number 1 and was affirmed as a final order on 5/26/2009, with respect to a workplace located at 27 w. 12th Street, Cincinnati, OH.
