29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: In the HDPE Number 1 Unit, for scheduled project work occuring around the ethylene purification section (C-212 B), the employer in its Procedure 11 - "Hazardous Energy Control" (09/2011), including Attachment "A" - "Lock, Tag Try Log Sheet", did not address the hazards of re-accumulation of hazardous energy (chemical, high pressure) in this process equipment while employees performed permitted work on or around this equipment.
