29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Location: 919 Collins AVE, Miami Beach, FL. On or about, May 1, 2013, at the above addressed job-site an employee was exposed to a 10-foot fall hazard while working from a roof without fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Location: 919 Collins AVE., Miami Beach, FL. On or about, May 1, 2013, at the above addressed job-site, an employee was exposed to a 10-foot fall hazard when the ladder used to access the roof was not extended 3 feet above the landing surface.
