29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material.  (a) Proto dept. On or about May 3, 2013, employees using unguarded Delta ShopMaster radial saw 301 were exposed to amputation hazards during jigs cutting operations.  (b) Proto dept. On or about May 3, 2013, employees using unguarded Delta ShopMaster radial saw 308 were exposed to amputation hazards during jigs cutting operations.  Abatement certification is required of this item in accordance with the requirements of 1903.19(c).

29 CFR 1910.213(h)(4): Radial saw was not installed in a manner so as to cause the cutting head to return gently to the starting position when released by the operator.  (a) Proto dept. On or about May 3, 2013, employees using unguarded Delta ShopMaster radial saw 308 were exposed to amputation hazards during jigs cutting operations where the cutting head had to be manually pushed back to return to its starting position.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about May 3, 2013, employees were connecting portable power tools and other equipment into square electrical boxes hanging from the ceiling which were not suitable for such installation.  Abatement certification is required of this item in accordance with the requirements of 1903.19(c).

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  (a) Proto Dept. On or about May 3, 2013, employees using electrical square boxes to plug in power tools and other equipment were exposed to electrocution hazard where knockouts were missing allowing access to energized components.  Abatement certification is required of this item in accordance with the requirements of 1903.19(c).
